Questions

A question is the action item that the user will provide a response to.

Your checklist can consist of as many questions as you need, and Auditflo offers a variety of customizations to ensure that you can create the most comprehensive questions as per your requirements.

Add a Question


Go to the Checklists tab from the left navigation bar on your screen. Click on the desired checklist from the list of checklists that will be displayed on the screen, or create a new checklist.
On the Checklist page, you can add questions.
  1. To add a question, click on the + Add Question button.


  1. You can also click on the  icon in the panel that appears when you hover above a particular question or section.


Using either of these methods, a blank question card will be created. Select the question type from the dropdown on the right side (for more details, visit: Question Types). Next, enter the question title, and set the response options (to learn more, go to the Response Fields section below).

Furthermore, the  image icon to the left of the question title, allows you to add an image to your question if needed. Click on the icon to insert any image file from your computer system.

Note: You cannot leave the title or response label of a question blank. Furthermore, you will be unable to create a new question until you specify these parameters for the previous question.

Once you create your question, when you click outside the question card, you will see the preview of the question in the Mobile Preview area (see video below for demonstration).

Response Fields

There are several fields in a single question, and you can configure them as per your requirements. Each field is described below.


1. Suggestions

The Suggestions magic wand allows you to choose from a few predefined response labels, as the answers to your question.

Note: This feature is applicable to the following question types: button, checkbox, dropdown, and radiobutton.

When you click on this wand, the list of response labels (organized in groups) will be visible. Sample labels include “Good, Bad, N/A”, “Yes, No, N/A”, etc. If you click on one of these options, these labels will automatically be added as responses to your question. You can edit or delete them, or add more response labels as needed.

Note: The Custom response type is selected by default, so if you do not choose one of the suggested options, you will have to create your own labels.



2. Response Labels

Response Labels contain the options you want to provide for the user to choose from, such that they can answer the question. You can have 1 or more response labels per question.

You can use the response labels from the Suggestions magic wand and modify them as per your requirements, if you dont want to create fresh response labels.
Note: This feature is applicable to the following question types: button, checkbox, dropdown, radiobutton, and text.



To create your own labels, simply type out the desired text labels of your choice into the Response Label textboxes. By default, once you start typing in a response label textbox, the next textbox will appear on the screen.


To change the order of response labels, hover over the left side of the label till the  dotted icon appears. Click on it. Now, you can drag the label up or down to modify the order.

To delete a response label, click on the label box and delete the text in it. The label will disappear automatically.

3. Auto Score Textbox

If Auto score is enabled using the checkbox below the checklist title, you can assign a score for each response label, which will be computed when the user clicks on the response while filling the checklist.

4. Response Label Image

If you want to add an image to your response label, select the black icon on the left of the label textbox. Choose an image file from your computer, and it will be added for that label.
If you want your respondent to provide an image in addition to the response label, select the image icon on the right of the label textbox. The icon will turn green , and when you publish your checklist, a  label will be displayed. The user executing the checklist will be able to upload an image once they have selected the response label.

5. Remarks Button

To enable the Remarks option, click on the remark icon on the right side of the response field.

Enabling this option will allow the users to enter their remarks while filling up the responses or choosing their options.


6. Create Action Button

For certain responses in your checklist, you may want to create a task for your user to perform. To do this, click on the Create Action button.

Choose the relevant action category from the available list, assign issue priority (Low, Medium, High, or Critical) depending on the urgency of the task, and add a description for that action. Once you publish the checklist, the  label will be visible next to the corresponding response in your checklist.

7. Required Field Toggle


If you want to make a question mandatory in the checklist (i.e., if you want to make it compulsory for the user to answer this question), click on the Required toggle button.
Note: This option is available with all the question types.
By default, this option is disabled. Clicking on the toggle button will enable it, making the question mandatory.

Once the checklist is published, mandatory questions will have a red asterisk next to the question title.

Once you are satisfied with your questions and responses, click on the Publish button to publish your checklist.

Now, your checklist is ready for use. You can create an Audit, and select this checklist as the Checklist Type on the Audit page, as shown below.

Edit a Question

You can modify questions in a checklist at any time. To do so, go to the desired checklist from the Checklists page.


Once your checklist opens, you will see all its questions. You can click on a question card to edit the question title/response labels, or click on the pencil icon of that question to edit as shown below.


Create a subquestion

You can create one or more questions under a single question, such questions are called subquestions. They will display on the user’s screen based on the the response they select while answering the parent question.

To create a subquestion:

  1. Create a parent question in the checklist using the + Add Question button.

  2. Add the response labels for this question.

  3. Click outside the question card, and now hover over the question you just created. You will see a blue plus button, as shown below, next to each response label.

  1. Set the question type and response fields for the subquestion. You can similarly create a subquestion under the other response labels. Once done, you will be able to see the question and the corresponding subquestions.



Subquestions may be hidden by default:


You can click on the icon to display the subquestions on your screen, and the  icon to hide the subquestions. Note that subquestions will not be hidden from the user’s view using these icons.

Duplicate a question

If you want to add a question which is similar to an existing question, you can duplicate an existing question and make the desired changes to the copy.

To create a duplicate question, simply click on the copy icon that appears when you hover over the question card. All the response fields will also be copied, thereby making it easier for you to modify the question!





Delete a Question

To delete a question, click on the trashcan icon on the right side of the card. A dialog box will appear.


If you are confident you want to delete the question and all the subquestions/response types within it, select the Delete option in the dialog box.

If you don’t want to delete the question, select the Cancel option to return to the checklist.
